The following are the forms you'll need to fill out and send to us to attend summer camps. Please fill these out and email them to:
Maryland Dept of Health and Human Hygiene Form
This form is required by the Maryland Dept. of Health and Human Hygiene. Your camper cannot attend an Impulse City camp if we do not have one of these on file. It's easy to fill out and does not require a doctor's signature.
Camper Inventory 2024
This form helps us get to know your camper. We put them in groups and schedule activities based on their interests. It also helps to know what strategies work for your child.
Waiver 2024
This form is required for your camper to attend Impulse City. While our camp is VERY focused on safety, accidents are possible. We do not want to be liable for actions that are beyond our control.
Medication at Camp
You only need to fill out this form if your child needs to receive medication while at camp. This would include inhalers, epipens, and any medication that your camper would need to receive from us during the camp day. It requires your pediatrician's signature.
